When you apply for a HELP loan, you must have read the relevant booklet for your loan (e.g. the Information for Commonwealth supported students and HECS-HELP booklet for a HECS-HELP loan). When you sign your Request for Commonwealth assistance form, you are declaring that you have read and understood the booklet and your obligations if you access the loan scheme. Your provider will supply you with a copy of (or link to) the relevant booklet for your study and how to complete the Request for Commonwealth assistance form.

Request for Commonwealth assistance forms are only available from your provider. There is a separate form for each loan scheme and students using FEE-HELP through Open Universities Australia must complete the Request for FEE-HELP assistance - Open Universities Australia form
These forms are produced in triplicate. One copy is for the ATO and the second copy is held by your provider. You should retain the third copy for your records.
Your provider may also allow you to complete the form online when you enrol. It will advise you of whether they offer this option at the time you enrol.
